Set only days after the first film, the story follows a high school baseball team, their cheerleaders, and their bus driver as they travel home after a victorious game. Their journey is cut short when their bus tire is mysteriously flattened. Stranded on a desolate highway, they soon realize they are being hunted by the Creeper—a ancient, bat-like demonic entity that awakens for 23 days every 23rd spring to feed on human body parts to regenerate itself.
The demonic, winged creature stalks the students, picking them off one by one based on the "scent" of their fear. Simultaneously, a farmer named Jack Taggart (played by Ray Wise) and his son embark on a vengeful mission to hunt the beast after it abducts Taggart's younger son.
